Chiefs of Ontario Non-Insured Health Benefits Patient Experience Survey

Please note the following:
1. This survey is not soliciting responses for any other NIHB program other than mental health services at this time.
2. Please do not take this survey if you do not live in Ontario.
3. Survey respondents will remain anonymous.
4. The survey has three short sections and a total of 12 questions. It should take you less than 10-15 minutes to complete.
