MCFN Council has approved a distribution of $1,500.00 for the 2025-2026 Community Wellness.

**2025-2026 DISTRUBUTION COMMUNITY WELLNESS APPLICATION INFORMATION**

THE FOLLOWING PROCEDURE WILL ONLY APPLY TO THE 2025-2026 COMMUNITY WELLNESS:

Application Forms and required documents will be accepted via email, mail, or in person at the Lands & Membership office (6 First Line) beginning April 1st, 2025 at 9 a.m. A mail slot is available at the office for after-hours drop off. Fax applications will not be accepted.

The 2025-2026 Distribution Community Wellness applications will be available in office, online at www.mncfn.ca, via mail or email starting April 1st, 2025 @ 9:00 a.m.

Members must use the new 2025-2026 application form for $1,500. Applications will not be accepted if other forms are used. Faxed applications will not be accepted.

Direct deposit (CANADIAN ACCOUNTS ONLY) is the preferred method of payment, your name must appear on your void cheque or direct deposit form NO EXCEPTIONS. If your name is NOT on your void cheque or direct deposit form, a cheque will be mailed to you. Not applicable if there are no changes to your banking information.

If you have requested a cheque, it will be mailed to you. Please ensure your address is up to date and complete.

Quotes/receipts/estimates must be dated April 1, 2025 or later.

Payments will be issued within 4 – 6 weeks, upon confirmation of your completed application(s).

Acceptable Valid ID (MUST PROVIDE A COPY OF THE FRONT & BACK OF 2 PIECES OF ID)

-Birth certificate
-Status card
-Health card
-Driver’s license
– Current Employee ID, with digitized photo
– Current Student ID, with digitized photo
-Firearms license
-Passport or Nexus card
-Provincial Photo ID card
